Our Team

As a Medicare-certified agency, we operate under the supervision of a Medical Director (typically an MD or DO) who heads the Professional Advisory Group to provide professional guidelines for the program.

Overseeing our routine activities are

The Administrator/CEO

The Administrator/CEO who is responsible for the general operational productivity of the agency, including employment and hiring, financial, compliance/regulatory with program, EEO, Affirmative Action and other administrative functions.

The Director of Nursing

The Director of Nursing, a registered nurse, whose primary functions are to select train and supervise a nursing staff of case managers, home health aides, coordinate skilled professional services for nursing, therapy and medical social services, and perform quality assurance functions.

Our first contact with a client can be made through several ways

  • Physician referral (patient requiring home health services)
  • Family referral
  • ‘Client Initiative – usually begins with request for information about our services.

Once we have thoroughly checked and verified all clients eligibility under our program’s guidelines (e.g., homebound status for Medicare clients),  physician ,and insurance information, we will begin the admissions process. This involves an in depth interview with the client to advise him/her of their rights and responsibilities HIPAA. protection of their information, advance directives, etc.), and obtain consent for service. This is followed by consultation with the client’s physician to obtain an order for services, a comprehensive assessment to determine a Plan of Care for ongoing service. Service must be reviewed and re-certified every 60 days to ensure the most current and accurate information.
